PHQ-9 — depression
Over the last 2 weeks, how often have you been bothered by any of the following?
1.Little interest or pleasure in doing things.
2.Feeling down, depressed, or hopeless.

5.Poor appetite or overeating.

Questions about the PHQ-9 & GAD-7
What is the PHQ-9?+
The Patient Health Questionnaire-9 (PHQ-9) is a nine-item screening tool used worldwide by GPs and mental-health professionals to gauge the severity of depression symptoms over the previous two weeks. Scores range from 0 to 27.
What is the GAD-7?+
The Generalised Anxiety Disorder 7-item scale (GAD-7) is a validated seven-question screener for generalised anxiety symptoms across the previous two weeks. Scores range from 0 to 21.
Is this self-assessment really private?+
Yes. The questionnaires are scored entirely inside your web browser on your device. Nothing is transmitted, saved to a database, or shared unless you deliberately choose to email your result to Rosemary or bring it to a session.
Is this a diagnosis?+
No. The PHQ-9 and GAD-7 are screening tools, not diagnostic instruments. They are a helpful conversation starter with your GP or a qualified psychotherapist — they cannot replace a professional assessment.
How long does it take?+
Most people complete both questionnaires in around three to five minutes.
How should I interpret my PHQ-9 score?+
0–4 minimal, 5–9 mild, 10–14 moderate, 15–19 moderately severe, and 20–27 severe symptoms of depression. Any positive answer to question 9 (thoughts of being better off dead or self-harm) warrants immediate follow-up regardless of total score.
How should I interpret my GAD-7 score?+
0–4 minimal, 5–9 mild, 10–14 moderate, and 15–21 severe anxiety symptoms. A score of 10 or higher is generally considered a reasonable cut-off for further assessment.
I scored high — what should I do next?+
Please talk to someone. You can book a first session with Rosemary, share your result by email, or speak with your GP. If you are in crisis, contact the Samaritans on 116 123 (free, 24/7), Pieta House on 1800 247 247, or dial 112/999 in an emergency.
I scored low but still don't feel right — is that valid?+
Absolutely. These questionnaires only measure specific symptoms over the last two weeks. Grief, relationship difficulty, trauma, life transitions and burn-out do not always show up in a PHQ-9 or GAD-7 score. If something is wrong for you, that is reason enough to talk.
Can I use this for my teenager?+
The PHQ-9 and GAD-7 have adolescent versions, but for under-18s we recommend sitting with your teen and, ideally, booking a teen counselling session so a qualified therapist can interpret the results in context.
Do I need to give my email or sign up?+
No. There is no sign-up, no account and no tracking of your answers. The 'Share this result with Rosemary' button simply opens your own email client with the totals pre-filled — you decide whether to send it.
Can I print or save my result?+
Yes. After you see your result, use the 'Print / save as PDF' button to keep a copy for yourself or to bring to your GP.
Is RC Psychotherapy available in Dublin and Wexford?+
Yes. Rosemary Chaney offers in-person sessions in Dublin and in Gorey, Co. Wexford, and secure online video sessions to clients anywhere in Ireland.
How much does a first session cost?+
Individual sessions are €85 in Dublin and €75 in Gorey or online (50 minutes). Couples sessions are €100 (60 minutes). See the Fees page for full details.
Where do the PHQ-9 and GAD-7 come from?+
Both questionnaires were developed by Drs. Spitzer, Williams, Kroenke and colleagues with an educational grant from Pfizer Inc. They are in the public domain — no permission is required to reproduce, translate, display or distribute (1999).
Should I retake the questionnaire later?+
Yes — many people re-take the PHQ-9 and GAD-7 every two to four weeks to track how they are feeling over time, especially once they have started counselling.
